We re hiring primary care nurse practitioners for multiple post-acute care opportunities throughout Upstate New York.

What to Expect:
Location: Multiple sites throughout Upstate New York
Schedule: Flexible weekday rounding (part-time and full-time options available)
Setting: Skilled nursing and post-acute care facilities
Role Type: W2, benefits-eligible
What You ll Do:
Deliver primary care to patients in post-acute settings
Manage both chronic and acute conditions with a patient-centered approach
Collaborate closely with interdisciplinary teams to improve patient outcomes
Build trusted relationships with patients, families, and facility staff
Partner with a supervising physician while working with autonomy
Who We re Looking For:
Licensed and certified nurse practitioners (FNP, AGNP, CNP, APRN)
Active New York license (or ability to obtain)
Compassionate, team-oriented, and motivated clinicians
Post-acute or long-term care experience is a plus, but not required
