The Family Medicine Physician provides comprehensive, continuous medical care to patients of all ages. This role emphasizes preventive care, diagnosis and treatment of acute and chronic conditions, and coordination of care across the healthcare continuum, delivering patient-centered, evidence-based services.
Essential Duties and ResponsibilitiesProvide comprehensive primary care services to pediatric, adult, and geriatric patients
Diagnose and manage acute and chronic medical conditions
Deliver preventive care including health maintenance, screenings, and immunizations
Perform office-based procedures consistent with training and privileges
Manage chronic disease conditions such as diabetes, hypertension, asthma, and cardiovascular disease
Coordinate care with specialists, hospitals, and ancillary services
Provide patient education regarding disease prevention, wellness, and treatment plans
Maintain accurate, timely clinical documentation
Participate in quality improvement, population health, and patient safety initiatives
Supervise and collaborate with advanced practice providers, residents, and clinical staff as applicable
Comply with all regulatory, accreditation, and professional standards
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
Completion of an accredited Family Medicine residency program
Board-eligible or board-certified in Family Medicine
Ability to obtain and maintain unrestricted state medical licensure
DEA registration or eligibility
Broad clinical knowledge across the lifespan
Strong diagnostic and clinical decision-making skills
Excellent communication and patient counseling abilities
Ability to manage multiple conditions in a primary care setting
Commitment to evidence-based practice and patient-centered care
Outpatient clinic with possible inpatient, obstetric, or urgent care responsibilities depending on practice scope
Call coverage as determined by practice or service needs
Ability to sit, stand, and move throughout clinic and hospital environments
Manual dexterity for examinations and office-based procedures
Visual and auditory acuity sufficient for patient assessment
