Family Medicine or Internal Medicine Post Acute Care Boca Raton, Florida
Position Highlights:
- Full-time position (4 5 days per week)
- Inpatient rounding in skilled nursing facilities, inpatient rehab, and long-term care
- Average of patients per day
- No primary night call
- Collaborate with interdisciplinary clinical teams and support smooth care transitions
- EMR: gEHRimed
Compensation & Benefits:
- Productivity-based compensation (Fee-for-Service); estimated range: $235,000 $278,000 annually
- No cap on monthly productivity income

Candidate Requirements:
- Board-certified in Family Medicine, Internal Medicine, or Emergency Medicine
- Active Florida medical license (or eligibility)
- DEA license (or ability to obtain)
- Prior post-acute care experience preferred
- Minimum of 1 year primary care experience
