Requirements · Anticipated OBHG deliveries – Unassignedpatients vs Private patients: 200 per year· Current LampD coverage model: 1OB covering all unassigned. · Reason for partnering with OBHG: Expand coverage and grow market share· Do you plan to open an OBED or keepa triage model: No· NICU level: Level 2· Anesthesia coverage model: Daily Onsite coverage with 24/7 Call coverage· Residency programs that wouldrotate through OBED/LampD and plan for OBHG involvement: Internal Med Program· Clinic/outpatientresponsibilities if any: Will see patients in OP setting New andRoutine Patients· Who will provide back up to OBHG inthe event of multiple emergencies OB/Gyn Call Schedule?: Employed OB’s· Gyn responsibilities: SurgeryConsults Outpatient Clinic· Will OBHG back-up private orhospital employed Certified Nurse Midwives or Family Practice Physicians: No· MFM coverage: Remote OPavailability through University of Kentucky

Schedule 24Hour, MHAS - clinic, call, and on call required.
