You will be the central point of care for patients, focusing on preventive medicine, chronic disease management, and health education in a collaborative, multidisciplinary environment.Key Responsibilities
- Perform comprehensive patient evaluations: medical history, physical exams, and ordering/interpreting diagnostic tests.
- Diagnose and manage both acute and chronic illnesses; develop personalized treatment plans.
- Provide preventive care, including immunizations, health screenings, and lifestyle counseling.
- Prescribe medications and other therapies, and monitor their effectiveness.
- Coordinate care with specialists and other healthcare professionals to ensure seamless patient follow-up and referrals.
- Educate patients about self-care, disease prevention, and healthy behaviors.
- Maintain accurate patient records using an electronic health record (EHR) system.
- Participate in quality improvement initiatives and adhere to clinical guidelines.
- Stay current with medical research, advances, and best practices.
- MD or DO degree.
- Board certification in Family Medicine, Internal Medicine, or equivalent.
- Valid state medical license.
- DEA registration (or equivalent) as required.
- Experience (or eligibility) in outpatient primary care.
- Proficiency with EHR systems.
- Strong communication, organizational, and interpersonal skills.
- Commitment to patient-centered, preventive care.
- Full-time, Monday to Friday.
- Standard business hours (daytime).
