We are looking for a skilled Nurse Practitioner to provide comprehensive primary care services to residents of assisted living facilities. The ideal candidate will have experience with the geriatric population and/or long-term care, and a commitment to delivering high-quality, patient-centered care.
Essential Duties and Responsibilities
- Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
- Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
- Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
Requirements
- Master s degree in Nursing and graduate of an accredited Nurse Practitioner program.
- Unrestricted New York State Nurse Practitioner license.
- Unrestricted Drug Enforcement Administration (DEA) permit to prescribe all medications commonly prescribed in the course of the practice.
- Eligible to provide reimbursable services and maintain provider status with Medicare andMedicaid.
- Minimum of two years clinical experience as an NP
Preferred Experience:
- Experience with eClinicalWorks (eCW) electronic health records.
