Physical Therapist (PT) – Home Health | Philadelphia, PA
MedAdventures is seeking a skilled Physical Therapist to join a home health team in Philadelphia, Pennsylvania. In this role, you’ll provide in-home care to patients, helping them improve mobility, strength, and independence while collaborating with nurses and other rehab professionals. OASIS experience is required.

Your Responsibilities:
- Provide high-quality PT care in patients’ homes
- Complete OASIS assessments and develop individualized care plans
- Collaborate with nurses, therapists, and interdisciplinary team members
- Maintain accurate, timely, and compliant documentation
- Educate patients and caregivers on exercises, safety, and recovery
Qualifications:
- Active Pennsylvania PT license
- Home health experience required, including OASIS documentation
- Strong clinical reasoning, communication, and teamwork skills
