Responsibilities: Full Scope Primary Care: Provide a full range of primary care services, including diagnosis, treatment, coordination of care, preventive care, and health maintenance. Clinical Expertise: Review patient history and conduct thorough physical examinations. Evaluate, diagnose, and provide appropriate treatment, referring patients to specialists as needed. Utilize quality clinical diagnostic skills in patient care, reviewing test results and recommending suitable management plans, including preventive health recommendations. Prescriptive Authority: Prescribe medications and formulate ongoing treatment and disease management plans. Relationship-Centered Care: Provide relationship-centered care, considering the patient s psychosocial and physical needs. Documentation and Communication: Collect and record patient information, maintaining complete and accurate medical records. Communicate effectively with the clinical team to ensure the total delivery of quality care. Educational Engagement: Participate in medical staff educational programs and meetings. Qualifications: Education and Training: Doctor of Medicine (MD), Doctor of Osteopathy (DO), or foreign equivalent. Completion of a three-year post-graduate medical training from a U.S. accredited residency program in Family Medicine or Internal Medicine. Experience: Seeking both experienced and newly trained providers. Licensure and Certifications: Board Eligible/Board Certified in Family or Internal Medicine. Current, unrestricted New York State license. Active Drug Enforcement Agency (DEA) registration or eligibility. Active Medicare/Medicaid Enrollment or eligibility. BLS or ACLS required. Communication and Technology Skills: Strong communication skills, both oral and written. Comfortable with Electronic Medical Record (EMR) systems and Microsoft Office.
