A Board Eligible/Board Certified Family Medicine/Internal Medicine Physician needed to join a growing Community Health group in Indiana. The role involves providing comprehensive outpatient care, managing patient treatment plans, coordinating referrals, and supervising advanced practice nurses and residents. Visa sponsorship and relocation allowances are available to support recruitment.

JOB RESPONSIBILITIES:
- Utilizes appropriate and effective patient care plans which includes adequate diagnostic procedures, therapies, use of consultation and referrals, and outcomes.
- Manages patients to assure continuity of care which includes adequate scheduling and tracking of return visits, completion of treatment plans, and follow-up of chronic problems, abnormal tests, and no show appointments.
- Provides whole-person care which includes appropriate
- referrals to the social worker, nutritionist, and mental health professionals.
- integration of biologic, social, psychological and environmental issues.
- Maintains adequate, concise, and legible documentation of patient care.
- Provides and documents appropriate periodic health examinations. (i.e. preschool vision exams, serum cholesterol, pap smears, etc.)
- Educates patients about their diagnosis, treatment, prognosis, medications, preventive care, etc.
- Utilizes appropriate infection control measures, including universal precautions.
- Supervises advance practice nurses in Clinic when assigned.
- Provides consultation and prescription signatures to advance practice nurses when needed.
- Oversees residents in the management of hospital patients.
- Writes daily progress notes in hospital charts.
- Takes phone calls about
