Experienced Primary Care MD/DO for SNF Rounds 2 Days/Week We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our team near Webster, NY. This part-time role offers the unique opportunity to provide vital, patient-centered healthcare services within a skilled nursing facility (SNF) setting, while enjoying the autonomy to set your own two-day weekly schedule. Key Responsibilities Conduct comprehensive primary care rounds at skilled nursing facilities twice a week. Diagnose, manage, and treat chronic diseases, illnesses, and complex medical conditions through well-developed care plans. Mentor and provide guidance and oversight to nurse practitioners on the team. Collaborate closely with facility leaders and an interdisciplinary team to enhance care coordination and uphold high-quality care delivery. Engage with patients, families, and staff to build strong, effective working relationships. Qualifications Minimum of 1 year of primary care experience is required. Active and unrestricted New York Medical License (or application in process). Active and unrestricted DEA/Prescriptive Authority (or application in process). Experience in post-acute care is advantageous but not mandatory.
