Join a mission-driven team providing comprehensive care for older adults in a PACE (Program of All-Inclusive Care for the Elderly) setting. We’re hiring a Physical Therapist who is passionate about functional mobility, fall prevention, and helping seniors stay safe and independent.
What You’ll Do:
Deliver physical therapy services in the PACE center, participant homes, and other community locations
Conduct evaluations, develop treatment plans, and coordinate care with an interdisciplinary team
Focus on prevention, independence, and quality of life — not productivity quotas
Participate in team meetings and provide input on long-term care strategies
What You’ll Need:
CA Physical Therapist license in good standing
Minimum 1 year of experience, preferably with older adults or in a community-based setting
Excellent interpersonal and collaboration skills
Comfort with home visits and real-world rehab solutions
