Job Function:

• Provide full-spectrum community health services to patients of all age ranges (0-100).

• Work within a collaborative and collegial environment as part of a Patient-Centered Medical Home (PCMH) practice.

• Offer outpatient care within a Federally Qualified Health Center (FQHC) / Community Health Center (CHC) system.

• Rotate through later nights and weekend coverage as part of standard business hours.

• Support the community health needs of diverse populations.

• Collaborate with healthcare professionals to deliver high-quality patient care.

• Eligible for student loan forgiveness through FQHC/CHC system participation.
