Position SummaryA healthcare organization is hiring Behavioral Health Therapists to support patients across primary care, integrated behavioral health, and psychiatric partial hospitalization settings. These roles provide assessment, brief evidence-based interventions, care coordination, documentation, and collaboration with interdisciplinary teams to improve clinical outcomes and patient experience. A sign-on bonus may be available for qualified candidates.
Key Responsibilities (role-dependent)Integrated Behavioral Health Primary Care
- Assess, triage, and manage patients referred by primary care providers for behavioral health and co-occurring medical concerns
- Deliver brief, evidence-based interventions (e.g., behavioral activation, problem-solving therapy, motivational interviewing)
- Use screening and outcomes tools; track follow-up and outcomes using registries and EHR documentation
- Provide same-day/urgent consultation support and crisis intervention as needed
- Support program development (e.g., pain, diabetes, insomnia, weight loss, stress) and maintain patient education resources
- Coordinate referrals to community resources and specialty behavioral health services
Psychiatric Partial Hospitalization Program (PHP) Therapist/Case Manager
- Provide group therapy, psychoeducational groups, and individual/family sessions
- Complete admission assessments, treatment planning, continuing care plans, and discharge documentation
- Support utilization review functions and ensure documentation supports medical necessity
- Participate in patient safety/behavior monitoring and therapeutic milieu support
- Strong assessment, crisis intervention, care coordination, and documentation skills
- Comfort working with interdisciplinary teams and high-acuity/stressful situations
- Proficiency with EHRs and common computer applications
Education
- Master s degree in Social Work or a related behavioral health field required (doctoral degree in psychology may be preferred for select roles)
Licensure/Certification
- Eligible for (or currently holds) appropriate state licensure as a behavioral health professional (e.g., LMSW/LLMSW, psychologist, counselor)
- Limited-license candidates may be considered if actively working toward full licensure within required timelines
- BLS required for select roles/within a specified timeframe
- Additional de-escalation/aggression management training may be required for PHP roles
Experience
- Integrated behavioral health/primary care experience preferred for primary care roles
- Prior mental health services experience preferred for PHP roles
