Setting: Skilled Nursing Facility (SNF)
We are seeking a licensed Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver therapy services focused on improving mobility, strength, and functional independence.
Key Responsibilities- Perform physical therapy evaluations and establish individualized plans of care
- Provide therapeutic interventions to improve patient mobility, balance, strength, and endurance
- Monitor patient progress and adjust treatment plans as appropriate
- Educate patients and caregivers on exercises, safety, and mobility techniques
- Collaborate with interdisciplinary healthcare teams to support patient outcomes
- Maintain accurate and timely documentation in the electronic medical record
- Ensure compliance with facility policies, rehabilitation standards, and patient safety guidelines
- Active California Physical Therapist license (must be current)
- Minimum 1 year of Physical Therapy experience preferred
- Skilled Nursing Facility (SNF) experience preferred
- Familiarity with NetHealth / Rehab Optima documentation system preferred
- Strong clinical judgment, communication, and time-management skills
- Bilingual skills not required
Candidates must meet the minimum qualifications and hold the required certifications to be eligible for application.
