We are seeking a qualified Speech-Language Pathologist (SLP) for a 13-week contract assignment in a Skilled Nursing Facility / Long-Term Care setting. This role focuses on evaluating, diagnosing, and treating residents with speech, language, cognitive, and swallowing disorders to improve overall communication and quality of life.
Key Responsibilities- Evaluate residents for speech, language, cognitive, and swallowing disorders
- Develop and implement individualized treatment plans
- Provide therapy services in accordance with physician orders and care plans
- Document evaluations, progress notes, and outcomes accurately and timely
- Collaborate with interdisciplinary teams to support resident-centered care
- Educate residents, families, and staff on therapy goals and strategies
- Active state Speech-Language Pathologist license (must be current)
- Minimum of 1 year of experience as an SLP
- Experience in Skilled Nursing Facility or Long-Term Care preferred
- Strong communication, documentation, and clinical skills
