Job Function:
Provide full-spectrum community health services to patients of all age ranges (0-100).
Work within a collaborative and collegial environment as part of a Patient-Centered Medical Home (PCMH) practice.
Offer outpatient care within a Federally Qualified Health Center (FQHC) / Community Health Center (CHC) system.
Rotate through later nights and weekend coverage as part of standard business hours.
Support the community health needs of diverse populations.
Collaborate with healthcare professionals to deliver high-quality patient care.
Eligible for student loan forgiveness through FQHC/CHC system participation.
Job Requirements:
Graduate from an accredited medical school and residency program in Family Medicine.
New York State Medical License.
Board Certification in Family Medicine by ABFM and/or ACOFP.
Current BCLS and DEA certifications.
Proficiency in Microsoft Suite and electronic health records (knowledge of Epic is a plus).
Bilingual in English and Spanish is a plus.
