Position Highlights
- Provide on-site primary care services within a senior living community in Greater Boston
- Deliver relationship-based care with a strong emphasis on continuity, access, and proactive management
- Practice in a low-volume clinical setting designed to support thoughtful, unrushed visits
- Typical daily schedule includes approximately 8 to 10 patient encounters
- Supported by an integrated care team, including nurse care managers, social workers, and community-based resources
- Collaborate with experienced physician leadership in a value-based care environment
- Role intentionally structured with built-in schedule flexibility
- Ability to work part-time or full-time, including options for two or three clinical days per week
- Board-certified Nurse Practitioner
- Clinical experience caring for older adult patients
- Prior exposure to senior living, assisted living, or home-based care preferred
- Comfortable practicing with a high degree of clinical autonomy
- Interest in longitudinal care and value-based care delivery
- Experience with Athena EHR is helpful but not required
