What You Will Do
As a Primary Care Nurse Practitioner, you will:

- Practice in a modern clinical setting, utilizing Advanced Access scheduling and strong clinical and administrative support systems.
- Collaborate within a multidisciplinary care team, offering both individualized patient care and population-based health services.
Qualifications
Advanced Practice Registered Nurse (APRN / FNP)
- Graduate of an accredited Master of Science in Nursing program with specialty as a Family Nurse Practitioner.
- ANA certification as a Family Nurse Practitioner.
- Connecticut APRN and RN licensure (required within 12 months).
- Bilingual English/Spanish skills strongly preferred.
- Required certifications include State of Connecticut Medical Licensure.

While this posting is for a Nurse Practitioner, physician candidates (MD/DO) are also welcome to apply. Physician qualifications include:
- MD/DO from an accredited medical or osteopathic school.
- Completion of an accredited family practice residency program.
- Board certification within 24 months of employment.
- Connecticut medical license or eligibility for licensure.
- Bilingual English/Spanish skills preferred.
