About the Role:

We are seeking an experienced Nurse Practitioner with strong clinical expertise in diabetes management to join our healthcare team on a flexible, part-time basis. Responsibilities:

• Provide patient education on diabetes management, including nutrition, glucose monitoring, and medication adherence.

• Support patients enrolled in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and weight management programs.

• Coordinate care and develop management plans for patients with complex diabetic conditions.

• Monitor patient progress, adjust care plans as needed, and maintain accurate clinical documentation.

• Communicate effectively with patients and multidisciplinary teams to achieve optimal outcomes.

Qualifications:

• Licensed Nurse Practitioner (NP) in good standing.

• Strong clinical experience in diabetes management or endocrinology (2+ years preferred).

• Excellent patient education and communication skills.

• Ability to work independently in a part-time, flexible schedule (20-24 hours per week).
