Hours: 5x8s | 40 hours guaranteed
Compensation:$2,500 per week
Start Date: ASAP or upon credentialing
Evaluate residents and develop individualized physical therapy treatment plans
Deliver skilled physical therapy services focused on improving mobility, strength, balance, and functional independence
Accurately document evaluations, progress notes, and discharge summaries per facility and regulatory guidelines
Collaborate with interdisciplinary teams including nursing, occupational therapy, speech therapy, and physicians
Educate residents, families, and caregivers on exercises, mobility strategies, and safety techniques
Maintain a high standard of patient-centered care in a long-term care environment
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Active Missouri Physical Therapist license (or ability to obtain)
Current BLS certification
Previous LTC or skilled nursing experience preferred
Strong communication, documentation, and clinical skills
