Practice Details:- Seeking a full-time locum Anesthesiologist starting May 2024
- Work at both Nampa and Boise locations
- Comfortable with Regional Anesthesia
- No Cardiac/Complex Peds/Transplants
- OB Coverage: Required for Nampa, Not Required for Boise
Credentialing Requirements:Ensure the following items are readily available for the provider at the time of acceptance to expedite the credentialing process:
- MD Diploma
- Internship/Residency/Fellowship Certificates
- All Medical Licenses
- DEA
- Board Certification
- Life Support Cards (BLS)
- Current and Previous Certificates of Insurance
- Drivers License
- Professional Color Photo/Headshot
- CMEs for the Past 2 Years
- Immunization Records (TB, Flu, HepB, Tdap, Varicella, COVID)
- References
