JOB SUMMARY:

RESPONSIBILITIES:
- Perform comprehensive assessments of residents' physical, emotional, and medical needs; monitor health status and adjust care as needed.
- Develop, implement, and regularly update individualized care plans in collaboration with physicians, therapists, and other healthcare professionals.
- Administer medications and treatments (oral, IV, topical, injectable, as permitted) and perform skilled procedures (e.g., wound care, IV therapy, specimen collection) per physician orders and facility policy.
- Monitor and record residents' vital signs, perform regular health checks, observe for changes or signs of concern, and respond appropriately to changes in condition or emergencies.
- Provide direct nursing care and support residents with activities of daily living, mobility assistance, hygiene, and other care needs when needed.
- Coordinate with the interdisciplinary care team (physicians, therapists, social work, CNAs/LPNs) to ensure comprehensive, cohesive, and resident-centered care.
- Maintain accurate, timely documentation of assessments, care provided, changes in condition, treatments, and communications in residents' medical records.
- Communicate with residents, families/POAs, and the care team regarding resident status, care plans, treatments, and any changes or concerns.
- Provide education and support to residents and their families regarding care, treatments, medications, and wellness.
QUALIFICATIONS:
- Valid, unrestricted RN license in Connecticut.
- Graduate of an accredited nursing program (ADN or BSN); background in long-term care or geriatric nursing preferred.
- Current CPR/BLS (or equivalent) certification as required by facility policy.
- Strong clinical assessment skills, compassion, and a patient-centered approach; excellent communication and interpersonal skills.
- Ability to work in a fast-paced long-term care or skilled nursing environment; reliable attendance and teamwork skills.
- Ability to perform nursing procedures (medication administration, IV therapy, wound care), monitor residents' conditions, and respond appropriately to changes or emergencies.
