  • Candidates must be board certified in Internal Medicine with a fellowship in Hematology and Oncology.
  • Physicians should hold an active medical license in Arizona (AZ) or be eligible for licensure through the Interstate Medical Licensure Compact (IMLC).
  • Required certifications include ABLS, ACLS, ATLS, BLS, and PALS.
  • The role requires a clean professional history for expedited credentialing, which typically takes about 45 days.
