Your Role & Impact 

The HIV/AIDS Case Manager facilitates a collaborative process to provide individualized case management services to HIV-positive clients that includes intake, assessment of client needs, care planning and psychosocial referrals, advocacy, partner services, HIV testing, and health education.

What We’re Looking For 

  • Bilingual in English and Spanish.     
  • Bachelor’s degree in Social Work, Public Health, or Social Sciences or three (3) years of work experience in a directly related field.
  • At least one (1) year recent experience in a healthcare setting. 
  • Knowledge and experience with public/private benefits and entitlement programs.
  • Experience with brief interventions and case management techniques.
  • Experience working with low income, diverse populations.           
  • Patient advocate; facilitator; negotiator skills.
  • Computer proficiency with Microsoft Office suite products (Outlook, Word, Excel, and PowerPoint) and electronic health record systems.   
  • Knowledge of issues and community resources related to HIV/AIDS, chemical dependency, mental illness, housing, health insurance, and other social services.

The pay range for this role is $25.30 - $35.42 on an hourly basis based on experience and internal equity.
