About the Clinician's Workday The nurse practitioner will deliver comprehensive family medicine care to elderly patients in a long-term care hospital setting, managing 16-20 patients daily with lower patient volumes and specialized focus on geriatric primary care needs. Clinical responsibilities include conducting thorough patient evaluations, managing chronic medical conditions prevalent in elderly populations, providing preventive healthcare services, and coordinating care with interdisciplinary teams. The clinician will work Monday through Friday from 8:00 AM to 4:30 PM in a 40-hour weekly schedule with no call requirements, maintaining accurate medical records and collaborating with full MD support staff including physicians, advanced practice providers, medical assistants, and registered nurses to ensure continuity of care and optimal patient outcomes within the long-term care hospital framework.
