The Registered Nurse in Ambulatory Neurology at Novant Health leads patient care by collaborating with healthcare teams to assess, plan, and implement individualized care plans. They conduct telephone triage assessments to determine patient needs, provide education, and escalate urgent conditions as necessary. The role requires current RN licensure, clinical judgment, technical proficiency in electronic medical records, and strong communication skills.
The Registered Nurse serves as a leader of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health or supports a peaceful death. The Registered Nurse utilizes best scientific evidence and compassion to assist the patient in navigating their health journey. The Registered Nurse accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures.
What We're Looking For:- Education: High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred.
- Graduate of an accredited school of nursing required. If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
- Experience: One year relevant experience, preferred.
- Licensure/Certification:
- Current RN licensure in appropriate state, required.
- ACLS, PALS, BAT and DOT; preferred.
As an RN in Phone Triage, you will serve as a critical clinical and technical resource for patients and care teams by:
- Conducting comprehensive telephone assessments to determine patient needs, acuity, and appropriate level of care
- Applying evidence-based triage protocols and sound clinical judgment to provide safe, timely guidance
- Delivering clear patient education, home-care instructions, and follow-up guidance via telephone
- Identifying urgent and emergent conditions and escalating appropriately to providers or emergency services
- Demonstrating strong technical proficiency in managing high call volumes for multiple providers, navigating multiple computer systems, and documenting accurately and efficiently in the electronic medical record
- Utilizing telephone systems, messaging platforms, and clinical software to manage patient calls, messages, and follow-up tasks
- Communicating and coordinating effectively with providers, clinical teams, and support staff to support continuity of care
- Upholding quality, safety, patient experience, and documentation standards in all interactions
