Job Description:
- Deliver skilled nursing care to patients in home health and hospice settings
- Perform comprehensive assessments and develop individualized care plans
- Administer medications, treatments, wound care, and pain management
- Educate patients and families about disease processes, medications, and end-of-life care
- Coordinate with physicians, case managers, and other interdisciplinary team members
- Maintain accurate documentation using EMR systems
- Manage a daily caseload and travel locally to patient homes
Education & Certification Requirements:- Active RN License in the state of Idaho – required
- BLS Certification (American Heart Association) – required
- Valid driver's license and reliable transportation – required
- ADN or BSN from an accredited nursing program
Experience Requirements:- Minimum of 1–2 years of recent experience in home health and/or hospice – required
- Strong clinical assessment and organizational skills
- EMR charting experience preferred
- Previous travel nursing experience preferred but not required
