Job Title: Home Health Registered Nurse (RN)
Location: Medford, OR and Surrounding Areas
Duration: 13 Weeks (Contract)
Shift: Day Shift | 8-hour shifts | 8AM to 4:30 PM

Pay: $2586/week gross pay
Key ResponsibilitiesProvide skilled 1:1 home health nursing care to assigned patients
Conduct up to 6 patient visits per day while meeting productivity standards
Perform comprehensive assessments, medication administration, wound care, and patient education
Complete accurate and timely documentation using EPIC/OASIS
Coordinate care with physicians, case managers, and interdisciplinary teams
Travel to patient homes within assigned geographic areas
Required QualificationsMinimum 1 year of recent skilled Home Health RN experience
Proven OASIS documentation experience (must be clearly noted if absent)
Ability to work independently in a home-based care environment
Reliable personal transportation required
Compliance with facility vaccination requirements (COVID primary series one booster)
Current CertificationsPreferred QualificationsStrong familiarity with EPIC charting system
Experience managing geographically grouped patient caseloads
Prior travel home health experience
