Advanced Practice Provider - Elder Care
Job Details:
• Full-time (1.0 FTE) NP/PA position providing gerontology-focused care to patients• Utilize advanced health assessment, knowledge, and decision-making skills both independently and in collaboration with physicians• Provide health care to individuals and families with emphasis on health promotion and disease prevention• Work onsite at patient residences, reducing burden on families and patients who would otherwise need to travel to clinics• Must be comfortable working autonomously offsite with collaborating physician partner available by phone• Schedule: 7:00 a.m. to approximately 5:00 p.m. daily• Involves after-hours on-call rotation, including weekends and holidays• Requires high level of customer service skills to establish positive relationships with patients, co-workers, and others• Education required: Master's or Doctorate degree in Nursing or Master's degree in Physician Assistant Studies from an accredited program• Must have current licensure as a Nurse Practitioner or Physician Assistant in Minnesota• Wisconsin licensure required within 3 months of hire• National certification in applicable area and current CPR certification required• Successful completion of credentialing process prior to practice• Ideal candidate will have experience as an RN or NP/PA with elder care background
Compensation:
Competitive hourly rates with weekly pay and malpractice included. May be eligible for additional compensation in the form of bonuses, quality incentives or production-based compensation.
