Job Description & Requirements

Required Qualifications:
- Evaluate and treat residents in skilled nursing and post-acute settings.
- Develop and implement individualized treatment plans to help residents achieve their highest functional potential.
- Collaborate with an interdisciplinary team to ensure seamless, coordinated care.
- Document accurately and timely in accordance with facility and regulatory standards.
- Participate in care conferences and contribute to ongoing quality improvement efforts.
License / Certification:- Active California Physical Therapist license (required)
- BLS/ACLS (AHA only)
