Presentation submission requirements:

Include an up-to-date CV, which shall include:

• Scope of practice in the CV job history section for each job
• Explain gaps on the CV (30-day or larger gaps)
• Copy of Board Certification(s) - If board eligible, list if the candidate is within 5 years of residency
• Submit a copy of a clean Sex Offender search - include a screenshot
• Supporting certifications such as DEA, BLS, ACLS, ATLS, NRP, PALS
• Procedures the provider is comfortable performing (for inpatient services)
• Copy of state license active status
• If obtaining a new state license for the provider, detail where in the process the provider is currently
• Other Active State License(s) - list the state, license number, and status
• Disciplinary Actions - must provide a detailed explanation with the presentation
• Malpractice History - supporting documentation
• Two current Vendor references forms filled out (within two years) supervisor and peer or two peer references
• Please speak to the RM if the candidate is new to the vendor and references have not been obtained

Facility Description:

Seeking a skilled CV Anesthesiologist to provide coverage from December 25 to January 5, with the potential for ongoing dates. The role involves working with multiple heart surgeons, performing anesthesia in CV cases, supervising CV CRNA(s), performing TEEs, and placing epidurals.

Details:

• 7a-3p with sporadic weekend call
• Duties include performing anesthesia in CV cases, supervising CV CRNA(s), performing TEEs, and placing epidurals
• Required Procedures: Facile with TEE and comfortable doing all CV cases, excluding transplants, including ECMO and thoracic cases. Able to perform epidurals.
• Setting: Level II Trauma Center, Strong CV program
• FTE: 2-3
• EMR: Cerner
• Support Staff: CV CRNA team and CV MD team
• Reason for Coverage: CV MD leaving the position and two others have gone to 75% of FTE
