American Traveler is seeking a Physical Therapist with 3+ years of experience and active state licensure for a day shift position in a rehabilitation therapy setting.
Responsibilities- Work in a long-term care and skilled nursing facility setting
- Day shift schedule
- Rehabilitation therapy focusing on maximizing residents’ functional abilities and independence
- Therapeutic modalities determined by individualized plan of care
- Collaboration with interdisciplinary team including physicians, nursing, and therapy staff
- Evaluate residents and establish plans of care including functional goals and frequency of therapy
- Consult and communicate plans of care and discharge needs with the healthcare team
- Document all evaluative and therapeutic interactions in the EMR according to department standards
- Provide education to residents and caregivers and participate in team meetings
- Supervise and collaborate with assistant-level staff and mentor new team members as assigned
- Maintain department cleanliness, follow infection control procedures, and participate in in-service education
- No radius requirement for local candidates unless otherwise specified
- Active Physical Therapist license in the applicable state; license required if applicable
- Minimum 3 years of relevant physical therapy experience
- Bachelor’s degree required; Master's or Doctorate preferred
- Basic Life Support (BLS) certification preferred
- Strong communication, documentation, and time management skills
- Proficient in Microsoft Office
- Ability to speak, read, write, and understand written English
- Must be able to lift up to 50 pounds, with frequent lifting/carrying up to 20 pounds
