JOB SUMMARY

Assume overall responsibility for the coordination of services during the youth’s placement. Coordinate the efforts of the multidisciplinary treatment team in the implementation of a comprehensive treatment plan for each child and their family assigned to the team. Coordinate the transition process and services for each youth. Serve as the main contact person with families and community partners. 

ESSENTIAL DUTIES

  • Coordinate the services a youth receives from the internal treatment team and the external team members.
  • Attend case presentation meetings and coordinate the Lad Lake team by assigning them to Extended Reach, ensuring the Lad Lake treatment goals align with the referral source's overarching goals.
  • Coordinate Monthly Treatment Plan Review documentation, ensuring all parties have completed their updates, and distribute the reviews to external team members and families.
  • Develop and implement the after-care and transition plan in consultation with the team.   Ensure aspects of the after-care plan and transition plan are in place prior to discharge.
  • Maintain central records as defined to be in compliance with Lad Lake, Inc. and licensing standards.
  • Provide training for staff as requested by the Residential Clinical Coordinator, Director of Residential Services, and the Human Resources Department.
  • In coordination with the Unit Therapist, prepare written summaries and provide testimony to juvenile justice court systems as required.
  • Assist with afterhours and/or weekend on call duties as needed by program. 

JOB REQUIREMENTS

Education/Training:  Bachelor’s degree in social work or other counseling field emphasizing child, adolescent, and family therapy.

Skills:  Demonstrated knowledge of child and adolescent development and behavior.  Demonstrated knowledge and skills in providing services in a trauma-informed way.  A proven ability to work with children and adolescents and their families who are often resistive to treatment investment.  A demonstrated ability to coordinate the treatment efforts of a multidisciplinary team toward common treatment goals.

Experience:  Formal academic training, supervision, and experience in working with adolescents and families and trauma-informed care.  On-the-job experience with a multidisciplinary treatment team in a hospital, residential, or group home setting. working with youth; Experience working in case management; Social work/Criminal Justice/Counseling related experience
