Title: Physical Therapist (PT) – Skilled Nursing Facility (SNF)
Location: Lancaster, CA
Duration: 13 weeks (26 weeks preferred)
Shift: Day shift | 5x8-hour shifts (08:00 – 16:30)
May include occasional weekend coverage as needed
Job Responsibilities and Required and Preferred Skills & Requirements:
Responsibilities:
Provide physical therapy services to patients in a skilled nursing facility setting
Evaluate patients and develop individualized treatment plans to improve mobility, strength, and functional independence
Deliver therapeutic interventions and monitor patient progress
Document evaluations, treatments, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary healthcare teams to support patient care goals
Required Qualifications:
Preferred Qualifications:
Minimum 1 year of experience in LTC and/or LTAC specialty
New graduates will be considered
Flexibility with assignment length and scheduling
