This full-time role requires board certification, relevant licenses, and experience with the elderly population in a collaborative interdisciplinary team setting.

Opportunity:
Nurse Practitioner - Geriatric Medicine
Job Details:
• Full-time position with an interdisciplinary PACE team providing comprehensive care to frail elderly adults
• Perform comprehensive medical assessments for program participants
• Develop and implement individualized care plans for seniors with chronic needs
• Provide acute illness evaluation and treatment as needed
• Participate actively in discharge planning processes
• Conduct home visits when clinically indicated
• Coordinate care across the full continuum of services offered by PACE •
Assist in the development and review of clinical policies and procedures
• Participate in provider on-call rotation
• Must be a Board Certified Nurse Practitioner (ANCC or AANP)
• Requires active Massachusetts NP license, DEA Registration, and CDS Certification
• BLS Certification required
• Must have experience with frail or elderly population
• Ability to travel within service area is essential
