On-site primary care duties, patient diagnosis/treatment, recordkeeping, quality improvement. - MD or equivalent - Current Indiana Physician License - Current DEA registration with prescriptive authority - Board Certified or Board Eligible in Family Medicine - Comfortable collaborating with NP, residents, medical students

Responsibilities:

• Provide primary care services to patients
• Diagnose and treat various medical conditions
• Collaborate with a team of healthcare professionals
• Maintain accurate medical records
• Participate in quality improvement initiatives

Qualifications:

• Doctor of Medicine Degree or its foreign equivalent
• Current and valid Physician License from the State of Indiana
• Current DEA licensure and prescriptive authority from the State of Indiana
• Board Certified or Board Eligible in related field
