This role is a per diem Physical Therapist position supporting PACE sites in Seattle and Kent, WA, responsible for evaluating and implementing physical therapy plans in various care settings. The PT collaborates with interdisciplinary teams and external caregivers to provide comprehensive care for elderly participants to maintain their independence. Candidates must hold a PT license in Washington and have relevant experience or advanced degrees in physical therapy, preferably with a geriatric focus.
Description
This is an on-call/per diem position supporting PACE sites in Seattle and Kent, WA.
The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA.

Required Qualifications:
- Bachelor's Degree in Physical Therapy
- National Provider BLS - American Heart Association within 30 days of hire
- Washington Physical Therapist License upon hire
Preferred Qualifications:
- Master's Degree in Physical Therapy or
- D.P.T. in Physical Therapy
- 1 year working with a geriatric population

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
