Geriatric Medicine Associates
Geriatric Nurse Practitioner-Lakewood/Westminster Co
The Practice: Independent group practice of 13 physicians and one geriatric nurse practitioner seeks a geriatric nurse practitioner to join our established outpatient geriatric clinic. The position will split the practice days between our Lakewood and Westminster offices. This position offers competitive salary and benefits. No call obligations. Epic EMR. Great work environment.

The Position:
Performs a comprehensive patient health assessment, including health history, annual wellness visits
Orders, administers and analyzes diagnostic tests.
Performs advanced technical procedures.
Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with physicians and other health care providers as appropriate.
Maintains accurate, legible and confidential records.
Trains patients and families on managing illnesses or injuries and disease prevention.
Develops and maintains a good working relationship with the healthcare team.
Qualifications (Education/Experience/Knowledge/Skills/Abilities):
Possess current, valid licensure as an Advanced Practice Nurse from the Colorado Board of Nursing from the Colorado Medical Board
Valid Prescriptive Authority in the State of Colorado
Current Drug Enforcement Administration (DEA) certification
Job Schedule: Full time
Workdays: 4 days a week.
Salary Range: 95-132k
