Nurse Practitioner (Home Visits) - $140k-$150k annual salary An innovative tech-driven healthcare company located in Middlesex County, NJ is currently seeking a Nurse Practitioner to provide Home Visits to patients. In this role, you will provide personalized, high-quality care to patients in their homes while enjoying a manageable patient load and the support of advanced technology. Prior experience in primary care or home care required. This is a full-time direct hire opportunity. Paid medical insurance and PTO offered.

Key Responsibilities:
- Deliver comprehensive, patient-centered healthcare services in the comfort of patients homes.
- Perform assessments, diagnose conditions, develop treatment plans, and provide follow-up care.
- Leverage advanced technology to streamline workflows, enhance patient care, and document visits efficiently.
- Collaborate with interdisciplinary teams to ensure continuity of care.
- Educate patients and families on managing chronic conditions, medications, and lifestyle adjustments.
- Provide compassionate and respectful care, fostering trust and strong relationships with patients.
Requirements:
- Active Nurse Practitioner (NP) license in NJ.
- Minimum of 1 experience in primary care, home care, or a similar setting.
- Exceptional interpersonal and communication skills.
- Tech-savvy with the ability to utilize digital tools for patient care and documentation.
- Passion for providing high-quality, personalized care.
Please note- The annual base salary range is $140k to $150k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
