Job DescriptionWe are seeking an experienced MDS Coordinator for our Skilled Nursing unit. This position involves conducting comprehensive MDS assessments for geriatric patients. The ideal candidate will demonstrate a strong commitment to patient care, ensuring high-quality assessments that adhere to regulatory guidelines.
Responsibilities- Conducting MDS assessments for geriatric patients.
- Collaborating with interdisciplinary teams to develop and implement care plans.
- Meeting regulatory compliance for documentation and patient records.
- Monitoring and updating patient care assessments as needed.
- Must have experience as an MDS Coordinator.
- 2 years of experience in MDS assessments preferred.
- Current RN license required.
- Certification in MDS training is mandatory.
