Job Description:
We're seeking a motivated Physical Therapist (PT) to join a dedicated team in a welcoming Skilled Nursing Facility located in Pahokee, FL. This facility offers a supportive environment, modern rehab equipment, and proximity to scenic Lake Okeechobee-perfect for outdoor enthusiasts looking for a peaceful Florida location.
Responsibilities:
Evaluate and treat patients with physical, emotional, or cognitive impairments
Develop individualized treatment plans and document progress in Net Health EMR
Collaborate with an interdisciplinary care team to promote optimal patient outcomes
Maintain 85% productivity
Requirements:
Active Florida PT license (or eligibility)
Previous SNF experience preferred
Strong clinical and communication skills
Available to start ASAP
