This full-time primary care role will include patient care rounding in a post-acute care setting in a skilled nursing facility (SNF) located in the Dover, Delaware, area
You will round 5 days a week with the flexibility to choose your rounding schedule
Primary care rounding in skilled nursing facilities
Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
Collaborate with clinicians, facility leaders and others regarding care and services
Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
Provide oversight, guidance, and mentorship to our nurse practitioners
Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
Minimum of 1-year primary care experience
Post-acute care experience a plus
Active and unrestricted Delaware license or application in process
Active and unrestricted DEA/prescriptive authority or application in process
Expected compensation is $210,529 to $270,529 annually with no cap on productivity income potential.
Sign-on bonus is available in months 1 and 13!
