Our client, one of the largest non-profit OPWDD organizations in New York, is looking for an RN Case Manager. This is a temp to perm role.

Job Function:

• Day Shift.
• 7am-3pm, 8am-4pm, or 9am-5pm.
• The RN Case Manager will oversee direct support professionals in a various home-care settings. Settings range from small houses to apartment complexes. Each Nurse Case Manager will be responsible for about 2 houses with about 5-10 patients in each, at most.
• This person will be expected to float between houses each day of the week as per required by patients' needs, but will only need to report to one home each day. They will not need to float among multiple house in a single day.
• This is a hands-off nursing position where the candidate will be directing DSP's on how to care for the patients and not doing the hands-on care taking work themselves. This role will require documentation on a regular basis.
• The client is very flexible with scheduling, allowing RN case managers to start/stop their day for personal reasons if/when necessary, as well as setting their own schedule on a weekly basis, within reasonable day-time hours.

Job Requirements:

• New York State RN License.
• Willingness to work with IDD population and be flexible with scheduling.
• Must have direct OPWDD experience or Sub Acute Case Management experience. 1 year preferred, 6 months minimum. Must have supervision or educator/training experience or in OPWDD.
