Hours : M/T/W 8-4:30 Th/F 6:30-3

Description:

Site provides light primary care, urgent care, vaccines, and lab services, Episodic, Immunizations, Flu Vaccination, CLIA waived testing (Rapid strep, glucose, urinalysis, cardio check, Influenza), Biometrics, Physicals, Labs (ordered internally and outside providers)

Employee and dependents covered under plan 8 years and up

Requirements:

Active Indiana license

BLS

NP Needed in Northwest Indiana

Shifts: 5am to 1:30 pm with an unpaid 30-minute lunch

Duties: General Primary Care, Acute Care, DOT Drug Testing

Light Occ Med

Ages 2 years and up (Employees and their dependents)

REQUIREMENTS

Active IN license

Active DEA

BLS certification

EMR - EPIC
