- Work withinterdisciplinaryteam,guidingtreatmentplansbasedonpatientneedsand preferences.
- Coordinatewithinterdisciplinaryteamtoestablishtentativedischargeplanandcontingency plans.
- Participateinplanningforandtheexecutionofpatientdischarge experience.
- Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
- Facilitateteamconferencesweeklyandcoordinatealltreatmentplan modifications.
- Completecasemanagementaddendumsandallrequired documentation.
- Maintainknowledgeofregulations/standards,companypolicies/procedures,anddepartment operations.
- Review/analyzecasemanagementreports,includingKeyCareIndicators,andplanappropriate actions.
- Understandcommercialcontractlevels,exclusions,payorrequirements,andrecertification needs.
- Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
- Meetwithpatient/familyperPatientArrivalandInitialVisitStandardwithin24hrs.of admission.
- Performassessmentofgoalsandcompletecasemanagementaddendumwithin48hoursof admission.
- Educatepatient/familyonrehabilitationandCaseManagerrole;establishcommunication plan.
- Scheduleandfacilitatefamilyconferencesas needed.
- Assistpatientwithtimelyprocuring/planningofresourcestoavoiddischargedelaysor issues.
- Monitorcompliancewithregulationsfororthoticsandprostheticsorderingand payment.
- Makeappropriate/timelyreferrals,includingdocumentationtopostdischarge providers/physician
- Ensureaccuracyofdischargeandpayor-relatedinformationinthepatient record
- Participateinutilizationreviewprocess:datacollection,trendreview,andresolution actions.
- Participateincasemanagementon-callscheduleas needed.
Qualifications
- Licenseor Certification:
- Must be qualified to independently complete an assessment within the scope of practiceofhis/herdiscipline(forexample,RN,SW,OT,PT,ST,andRehabilitation Counseling).
- Iflicensureisrequiredforone'sdisciplinewithinthestate,individualmustholdan active license.
- MustmeeteligibilityrequirementsforCCM®orACM™certificationuponentryintothis position OR within two years of entry into the position.
- CCM®orACM™certificationrequiredORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position.
- Minimum Qualifications:
- For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthanan associate degree.
- Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelor's degree and graduate degree is preferred.
- 2yearsofrehabilitationexperience preferred.
