• Perform in-home evaluations of speech, language, cognition, voice, and swallowing

• Develop and carry out individualized treatment plans aligned with physician orders

• Educate patients and caregivers to support progress between visits

• Collaborate with nurses, therapists, and physicians to ensure continuity of care

• Maintain accurate, timely documentation

• Promote patient safety and help reduce avoidable hospitalizations

• Master’s degree in Speech-Language Pathology from an accredited program

• Active state SLP license (or Educational Limited License with supervision)

• Prior experience preferred; home health or hospice experience is a plus

• Strong organizational skills and ability to work independently

• Reliable transportation and valid driver’s license
