------------------------------------------------------------------------------------------What You’ll Be DoingPerform in-home evaluations of speech, language, cognition, voice, and swallowing
Develop and carry out individualized treatment plans aligned with physician orders
Educate patients and caregivers to support progress between visits
Collaborate with nurses, therapists, and physicians to ensure continuity of care
Maintain accurate, timely documentation
Promote patient safety and help reduce avoidable hospitalizations
Master’s degree in Speech-Language Pathology from an accredited program
Active state SLP license (or Educational Limited License with supervision)
Prior experience preferred; home health or hospice experience is a plus
Strong organizational skills and ability to work independently

Reliable transportation and valid driver’s license
