This position is for a Primary Care Physician specializing in geriatric medicine to provide comprehensive care for frail older adults within a team-based environment. Responsibilities include assessment, care planning, management of chronic and acute conditions, coordination across multiple care settings, and participation in quality and education initiatives. Candidates must hold an MD/DO, be board certified or eligible in Internal Medicine, Family Medicine, or Geriatrics, hold an active state license and DEA, and have at least one year of experience with elderly populations.
