In this position, you will:
Provide high-quality care to geriatric patients in long-term care, assisted living, or similar settings
Conduct assessments, develop treatment plans, and collaborate with an interdisciplinary clinical team
Deliver compassionate behavioral health support when needed
Maintain strong communication with patients, families, and care teams
Licensed Nurse Practitioner/Physician Assistant (state licensure required or obtainable)
Behavioral Health experience preferred
Geriatric experience helpful but motivated new graduates are welcome
Compassionate, patient-focused, and team-oriented
