Annual Salary: $135k-$185k (varies depending on years of experience)
A community-focused behavioral health organization is seeking a dedicated Psychiatric Nurse Practitioner to provide psychiatric care and medication management to adult men residing in transitional housing.
Key Responsibilities:
- Conduct psychiatric evaluations and manage medication regimens for adult male clients with complex behavioral health needs.
- Support treatment adherence and coordinate care, including follow-up appointments and referrals.
- Collaborate with shelter staff and case managers to ensure continuity of care and crisis intervention.
- Provide hands-on support to clients, including motivational engagement and psychoeducation.
- Maintain accurate documentation and contribute to care planning in coordination with a collaborating psychiatrist.
- Travel between two fixed shelter locations weekly; mileage and tolls reimbursed.
Requirements:
- Licensed Psychiatric Nurse Practitioner (PMHNP) in New York State.
- Minimum of 2 years experience working with adults with serious mental illness.
- Experience in shelter-based care, ACT/IMT teams, or similar community mental health settings strongly preferred.
- Must have access to a personal vehicle and be comfortable traveling between two boroughs.
- Strong interpersonal skills and a commitment to working with underserved populations.
