Key Responsibilities:
- Provide telemedicine consults for nursing home residents in your specialty.
- Conduct initial in-person assessments as needed.
- Collaborate with nursing home staff, primary care providers, and other specialists to develop and implement individualized care plans.
- Review patient histories, diagnostic tests, and lab results to provide expert recommendations.
- Ensure timely documentation of consults and recommendations in the medical record.
- Board-certified or board-eligible in your specialty.
- Active NY medical license or eligibility for licensure.
- Strong communication skills and ability to collaborate in a multidisciplinary team.
- Prior experience in a nursing home or long-term care setting is a plus.
- Flexible telemedicine schedule, with some required on-site visits.
